PRE-PROPOSAL CONFERENCE:  There will be a pre-proposal conference for this sourcing event at 2:30 PM PST on Wednesday, July 10th at the Gladys McCoy Building, Room 155, 619 NW 6th Ave, Portland, OR 97209. There will be a call-in option. Call-in details will be provided in an addendum to this event no later than July 9th.  Attendance is: Optional Call-in number: (877) 336-1828 Access code: 1042243 INTRODUCTION The Multnomah County Health Department (“County”) is seeking proposers from whom it may purchase Occupational Radiation Exposure Monitoring services, as designated by the County. Occupational x-ray monitoring services are utilized at multiple locations within Multnomah County, including eight (8) dental locations, one (1) animal care services location, and one (1) Medical Examiners location. These services are to ensure that x-ray equipment is operating safely and that employee/patient exposure to radiation is minimized.  SCOPE OF WORK The awarded supplier shall provide the following: Provide the County with monthly x-ray radiation monitoring services and the necessary supplies: Personal and area Occupational Radiation Exposure Badges Mounting accessories for both personal and area monitoring badges Shipping manifests for all items Preprinted return labels. Badges and Supplies can be delivered to the Gladys McCoy Building 619 NW 6th Ave 8th Floor Lab; Portland Oregon 97209 Occupational Radiation Exposure Report to include the following information: Wearer Number Personnel name or other identifying designation Badge type Monitoring period Describes equivalent in millirems per periods; including current, quarter to date, year to date and lifetime to date; for deep, eye, and shallow Complete description of all indices and terms used in the report Turnaround time on Occupational X-ray Exposure Analysis shall be no longer than sixty (60) calendar days. Occupational X-ray Exposure Analysis results will be mailed to the Laboratory/X-ray manager. Monitoring results should clearly indicate if the badge wearer or area have above normal radiation exposure. METHOD OF AWARD The County intends to award one contract. Award, as determined by the County, will be made to the highest scoring Proposer whose Proposal the County determines is most advantageous to the County based on the described evaluation process and evaluation criteria. Multnomah County strongly encourages the participation of Minority-Owned, Women-Owned, and Emerging Small Businesses and Organizations in providing these services. ESTIMATED PURCHASES The County estimates approximately 65 badges will be required in rotation for radiation monitoring. The County does not guarantee that any minimum amount of services will be purchased. METHOD OF CONTRACTING A Services Contract form is provided in the Buyer Attachments page showing the standard terms and conditions.  Carefully review the terms and conditions of the Contract. Additional Contract terms related to this procurement, if any, are set out below in the section entitled Term of Contract.  TERM OF CONTRACT The contract term shall be five (5) years. The County shall have the right to terminate the contract on any contract anniversary date. CONTRACT NEGOTIATIONS The County will initiate contract negotiations with the responsive and responsible Proposer with the highest scoring proposal. Multnomah County may, at its option, elect to negotiate general contract terms and conditions, services, pricing, implementation schedules, and such other terms as the County determines are in the County’s best interest. If negotiations fail to result in a contract, the County reserves the right to terminate the negotiations and initiate contract negotiations with the next highest scoring responsive and responsible Proposer. This process may continue until a contract agreement is reached. INSURANCE REQUIREMENTS The insurance requirements are set forth in Exhibit 2 of the Services Contract located in the Buyer Attachments page.  Please review these requirements carefully. MINIMUM PROPOSAL SUBMITTAL REQUIREMENTS At the time of proposal submission proposers must meet the following minimum requirements. Failure to provide any of the required documents or meet any of the below requirements shall result in rejection of the proposal. The Proposal response must be received by the Multnomah County Purchasing no later than 4:00 P.M. local Portland time on the proposal submission deadline. Proposer Representations and Certifications - The Proposer must certify that they agree to the Proposer Representation and Certification terms in the Prerequisite page of the Sourcing Event. At the time of Contracting proposers must meet the following minimum requirements. Failure to provide any of the required documents or meet any of the below requirements shall result in cancellation of the contract. Proposers must be legal entities, currently registered to do business in the State of Oregon (per ORS 60.701). Proposers must submit verification that all insurance requirements are met.
